TOD BECKER, RN is a RN Navigator on the Tandem365 team and specializes in assisting participants with routine and urgent needs. As a Case Manager, Tod enjoys finding options and accelerating care for your admitted concerns. Functioning as an essential care coordinator, he will work to identify and minimize both current and predicted risks to your success at home. Tod is a fierce advocate for good communication and collaboration with primary care physicians, specialists, family and community supports. Tod has experience with nursing, education, “mercy needs”, and working with patients and families in a local hospital, where he worked for 10 years before joining the Tandem team. Tod has worked in the ICU, oncology, cardiology, nephrology, and med- surg floors. He holds an additional certificate in Gerontology with his nursing degree. Having children with birth defects contributes to Tod’s strong sense of compassion and advocacy for success at home. Tod rounds out his days and time managing a small blueberry farm, playing music, working on construction projects and church ministries.
Shelly RichardsonRN Navigator
SHELLY RICHARDSON, RN, is a Registered Nurse working with Tandem365. In this role, Shelly coordinates all aspects of Home Case Management including physical and emotional assessment, coordinating care with medical providers and facilitating necessary resources for identified needs to allow participants to remain in their home and avoid unnecessary emergency and hospital visits. Shelly holds an Associate Degree in Nursing from Montcalm Community College. Shelly has been a nurse for 24 years and has worked at a hospital with medical surgical patients and long-term care. She has also worked at a physician office with endocrinology. She has been trained and certified in wound care and has been working with Homecare and Case management for 20 years. Shelly has been working with Tandem365 since 2014.
Stefanie McCallumRN Navigator
STEFANIE MCCALLUM, RN has worked in health care for over 20 years. She worked as a certified nursing assistant in long-term care, home health care, and private duty before returning to school and earning her Bachelor of Science in Nursing in 2008 at Grand Valley State University. Before coming to Tandem365 in 2016, Stefanie worked in a medical/surgical critical care setting. In her role as an RN Navigator, she enjoys empowering her participants to manage their health care as independently as possible and being a staunch advocate for her participants’ goals and preferences. She has a real passion for education and teaching, whether it’s educating a participant and their family or passing her knowledge on to future generations of nurses. Stefanie is married with three children. She enjoys music, going to concerts and festivals, and is a voracious reader.

ANNALEE FOWLER, RN is an RN with Tandem365. In this role, AnnaLee provides high level nurse case management to medically complex individuals in their homes. AnnaLee holds a BSN degree from Grand Valley State University, graduating in 2001. AnnaLee has many years of experience working as a surgical and post operative ICU nurse, followed by ten years as a specialized clinical nurse for a busy Cardiothoracic Surgery office. She has special interest in treatment of heart failure and diabetes.

SHERRY GOSSET, RN is a Registered Nurse with Tandem 365. In this role, Sherry coordinates all aspects of Home Case Management including physical and emotional assessment, coordinating care with medical providers, facilitating necessary resources, for needs identified to allow participants to remain in their home and avoid unnecessary emergency and hospital visits. Sherry holds a diploma in Nursing from Blodgett School of Nursing. Sherry has spent 33 years in varied specialties, such as Mother Baby, Pain management, Medical Surgical, Oncology, Orthopedics, Neurology, Progressive Care and Home-based Case Management. Sherry has been with Tandem 365 for 1 year, assisting participants to navigate the health care system and advocating, on their behalf, for their needs.
Amy MullerRN Navigator
AMY MULLER, RN received her Bachelor of Science degree in nursing from GVSU in 2001 and has worked full time as an RN ever since. Starting at Blodgett Hospital, Amy worked as an orthopedic/med-surg nurse for 5 years, followed by 2 years in the primary care setting. For the next 7 years, Amy devoted her nursing career to mental health, working as a case manager in residential behavioral health programs, staff nurse in addictions, and as a nursing supervisor in the inpatient psychiatric hospital. Amy has been with Tandem365 since 2015. She values the longevity of the relationships she builds with her participants and considers it an honor to be able to provide support, education, and advocacy for her patients. In her spare time, in addition to raising three school age daughters, Amy loves musicals, movies, and spending time with her many pets.

JENNIFER LECHNER, RN works with Tandem365 to provide medical case management. Jennifer strives every day to ensure patients are well supported, educated in their health and strives to fight as her patient’s health care advocate. Jennifer has a deep passion for working out in the community and truly enjoys caring for patients in the comfort of their own homes. Jennifer recognizes the challenges and struggles that the aging population may face and she makes it her highest priority to ensure her patients are well supported, safe and knowledgeable in all aspects of their mind, body and spirit. Jennifer has been a Registered Nurse for 12 years and has worked in the health care system for more than 15 years. Her past experiences include emergency, urgent care and cardiac specialties. Jen and her high school sweetheart Joe have two children, Charley (6) and Evelyn (3) and a wild and crazy Labrador Retriever named Tucker. She enjoys fishing and kayaking and spending time with her family. Jennifer graduated from Grand Rapids Community College with her Associates Degree in Nursing in 2008. She went on to graduate from Chamberlain College of Nursing in Chicago with her BSN.

KATHY COBB, RN is a Nurse Navigator with Tandem365. In this role, Kathy works closely with the multidisciplinary team with a focus on home case management; providing assessments, education, medication management, and working with the medical team on needed interventions. The overall goal is to prevent unnecessary hospitalizations and assist each participant in their personal health goals. Kathy has been a registered nurse for 20+ years with expertise in the field of skilled home health. She has held many different positions as a clinical community nurse, preceptor, clinical manager, director and administrator. She graduated with honors from William Paterson University in New Jersey with a Bachelor of Science in Nursing. She obtained an 18-month Leadership Certification through Mercy Hospital and Saint Louis University. She has moved and worked in many states but is happy to be home in Michigan. Carrie Toland has 10 years of expertise in nursing. Before joining Tandem365 in 2019, Carrie worked as an Emergency Department nurse for 3 years where she has extensive knowledge in multiple disease processes. Prior to the Emergency Department, Carrie worked as a triage nurse for a Primary Care practice and as a Hospice nurse where she has experience with end of life care. Carrie is compassionate and is dedicated to her participants.

NIKI ISAAK, RN, BSN is a Nurse Navigator with Tandem365. She holds a bachelor’s degree in Nursing and Science. In this role at Tandem365, Nicole assists in the health care management of participants in the community with the goal of prevention of hospitalizations and falls. Nicole has a diverse background in nursing with over 11 years of expertise. Before joining Tandem365, Nicole worked for a local agency as a home care nurse for a short time before being promoted to a Clinical Manager. Prior to this Nicole worked in mental health and has extensive knowledge in Psychiatric Nursing and mental illness. Nicole is passionate about caring for others and is very dedicated to her participants. She works diligently to navigate her participants through the challenging health care system, honoring each person’s goals and individual needs.
Andrew CrowRN Navigator
ANDREW CROW, RN received his Bachelor of Science degree in nursing from Andrews University in 1996 and has worked full time as an RN ever since. Starting at Bronson Hospital, Andrew worked as a med-surg and ICU float nurse for 3 years, followed by 12 years in the emergency care setting. For the next 2 years, Andrew devoted his nursing career to neuro injuries, working as a case manager in neuro ICU followed by 5 years as a clinical care coordinator at Mary Free Bed. He then spent 2 years as the Nursing supervisor for Hope Network Neuro Rehabilitation. Andrew has been with Tandem365 since December 2019. He is looking forward to the relationships he will be able to build with the participants. He loves the ability to be an advocate for his participants and their families. In his spare time, Andrew enjoys painting and reading. He also has been learning to work out again.

Adam Strot is an Integrated Care Paramedic for Tandem365. He has over 15 years of experience as a Paramedic including jobs such as Supervisor and training officer. He has been involved with integrated care programs for the past 5 years both at Tandem365 and in Ft. Worth, Texas. Adam’s job at Tandem365 is to work with you, the participant, your primary care doctor, and any specialists that you see to ensure that you get the appropriate care at the appropriate time. Adam strives to help Tandem365 participants manage common medical issues such as Congestive Heart Failure, COPD, and Diabetes at home through education and in-home intervention when needed. Adam is married and has 2 children. In his time off he enjoys spending as much time outside and spending as much time with his family as possible

CHRIS HABEL, EMT-P, is an Integrated Care Paramedic with Tandem365. In this role, Chris helps provide high level medical case management, which includes working with the primary care physician and other providers to come up with the most appropriate plan of care, while also providing urgent in-home assessments and treatments when needed. Before joining Tandem365 in 2018, Chris worked for 11 years with other ambulance companies in Michigan and Texas as a critical care paramedic and a field supervisor. Chris enjoys helping participants manage the complexity of their medical needs.

MARILEE BRADFORD, EMT-P is an Integrated Care Paramedic Case Manager with Tandem365. In this role, Marilee works with physicians, nurses, social workers, medical assistants and many other professionals to coordinate the care of Tandem365’s participants. Marilee has more than 22 years of experience as a paramedic. Before joining the Tandem365 team she worked at a local ambulance company providing advanced life support and critical care to the people of her community. She also held leadership roles in the EMS system and once worked as a full-time firefighter. Drawing on her 22 years of experience as a paramedic, Marilee now focuses mainly on case management and the coordination of care in order to keep Tandem365 participants safe in their homes for as long as possible.
Eric DanielsICP PRN
Eric Daniels, EMT-P, has been involved in emergency medicine for over 26 years. He started his career as a Cadet firefighter in rural Kansas where he obtained his EMT. In 1996 he attended Davenport University for his Paramedic Degree. Eric has served in many roles through-out his career including as a Field Training Officer, Tactical Paramedic, and Disaster Response Team member. He continues to be heavily involved in advancing the careers of other responders by being and instructor coordinator, teaching various specialized courses in EMS education. Eric also is a member of Life EMS’s CISM team supporting other rescue workers in need. At Tandem 365 Eric uses his knowledge and experience as our Urgent Response Integrated Care paramedic. When Eric is not serving our participant’s, he serves his community as Firefighter and active at his church.

Jason McMartin, EMT-P, has been a paramedic for 23 years. Jason has worked as a paramedic on the ambulance and in the urgent care setting. Jason taught paramedic for five years for a local school. Jason has been with Life EMS for 4 years as a dispatcher, paramedic and TANDEM365 PRN ICP. Jason’s wife is also a paramedic with Life EMS. They have two children, ages 14 and 21 and they enjoy camping and traveling in their free time.

Alex Burgnon is an Integrated Care Paramedic Case Manager with Tandem 365. Alex has 10 years of experience as a paramedic, including roles as a critical care paramedic and field training officer. Alex’s job is to help manage all aspects of his participant’s care and to coordinate with physicians, nurses, social workers and medical assistants in order to formulate the best care plan. Alex will also provide in home assessments and treatments as needed. In their free time, Alex and his wife make it a point to travel as much as possible with an emphasis on the outdoors and exploring other cultures

Hiedi Johnson, EMT-P, is an Integrated Care Paramedic with Tandem365. She has worked in the Kent County EMS system for over 20 years, including time spent as a Critical Care Paramedic and a Field Training Officer. In her role with Tandem365, Hiedi works with primary care physicians, any specialists, and the participant to help provide the best, most appropriate care, in a timely fashion. Teresa Toland, CEO is responsible for a startup business where no blueprint or conditions of participation exist. Her 25+ years of experience in community health care has prepared her for her role in this new initiative. Teresa has spent the past eight years getting to know the health care systems and payers in our community and connecting in a positive way. The success of Tandem365 relies strongly on the ability to understand what health care systems, physicians, payers, patients and caregivers need today and being prepared for a dynamic healthcare industry. Teresa firmly believes that healthcare will continue to fail in the effort to delivery high quality care while at the same time manage the cost of delivering that care. Healthcare providers are confronted every day with situations that challenge them to balance patient needs, insurance demands, and regulatory restrictions and yet produce “quality” driven outcomes. Many American’s will be left on the sideline struggling to make sound choices while attempting to understand a very fragmented complex health system.
Michael McCully, D.O.Medical Director
Dr. Michael McCully serves as TANDEM365’s Medical Director with over 30 years of experience in both medical and academic sectors. Dr. McCully earned his undergraduate degree at St. Edward’s University in Austin, Texas and his medical degree from Kirksville College of Osteopathic Medicine in Kirksville, MO. He is a long time member of the American Osteopathic Association and the American College of Osteopathic Internists among several others. He also has been awarded many honors including Physician of the Year from Dallas-Ft Worth Medical Center and Internal Medicine Faculty Educator of the Year. Dr. McCully’s comprehensive experience with diverse populations compliments his ability to guide and lead TANDEM365’s clinical initiatives
Tara WilcoxDirector of Clinical Services
Tara Wilcox, RN, BSN, WCC is the Director of Clinical Services at Tandem365, a community paramedicine and complex medical case management program that currently serves over 1000 older adults in Kent, Ottawa, Allegan, and Kalamazoo counties. She obtained her BS in Nursing from Grand Valley State University in 2004. Tara has spent most of her career serving the aging population in various roles, but post-acute care in the home setting is her true passion. Prior to her current role with Tandem365, Tara worked to obtain several advanced certifications in wound management, treatments, and debridement so that patients could have access to advanced wound care options in the comfort of their own home. Tara’s current role fits her true passion for the elderly population due to her main interests in bettering the quality and access to care options available to seniors, assisting the at-risk elderly to age in place, and robust advocacy for improvements in navigating older adults through the complex healthcare system.
Phil FennemaDirector of Operations
Philip M. Fennema, EMT-P, CCP is the Director of Operations for TANDEM365. TANDEM365 is an innovative organization designed to provide better care, better health, at lower cost. TANDEM365’s primary purpose is to allow older adults to age in place and safely remain in their homes while avoiding unnecessary hospitalizations. TANDEM365 operates in the space where a medical case management program intersects with a community paramedic program. Philip’s 17 years’ experience in emergency medicine provides a unique insight into the challenges an organization like TANDEM365 faces. Philip recognizes the challenges older adults with complex medical conditions face and firmly believes TANDEM365 can provide innovative solutions. Through engagement with local, regional, and state level emergency medical service agencies, Philip serves this integrated care model by ensuring non-traditional utilization of EMS services is effective.
Katie HuizingaClinical Manager RN
Katie Huizinga, RN is currently the Clinical Manager for the nursing team at Tandem365, a complex case management and community paramedicine program that serves over 1000 patients in Kent, Ottawa, Allegan, and Kalamazoo counties. Katie’s role at Tandem365 has been pivotal for improving the access to care and quality of life for medically complex older adults. The majority of Katie’s experience has been in the emergency room setting, but after joining Tandem365 in 2015, she discovered her true passion for the aging population in the post-acute setting. Katie’s natural strengths have solidified her value to the role of case management and for the leadership role that she holds. Katie strives to be innovative and challenge the complexities of healthcare that are present in today’s system and that pose as barriers for those in advanced age.
Laura KasperskiClinical Manager RN
Laura Kasperski, RN is currently the Clinical Manager of the nursing team at Tandem365, a complex case management and community paramedicine program that serves over 1000 complex older adults in Kent, Ottawa, Allegan, and Kalamazoo counties. Laura obtained her BS in nursing from Mount Mercy College in Cedar Rapids, Iowa and quickly knew that she had a passion for the elderly population. Laura gained pivotal experience in both the ER and ICU settings prior to taking on more leadership roles that specifically influenced the care and quality of life of the aging population. Laura has received additional training in Polarity Thinking through the Interprofessional Institute for Polarity ThinkingTM in Healthcare which she has used to lead and guide her team though difficult situations and more easily adjust to change. Laura continues to strive to be a true change agent and believes that challenging the status quo in healthcare will lead to improvements in collaboration amongst providers, enhance access and quality of life for patients, and impact the overall healthcare system in a positive way.
Desiree ThompsonClinical Manager Care Transitions
Desiree Thompson, RN is a Clinical Manager of Care Transitions who works with medically complex older adults to help them age in-place, safely, at home. Desiree believes that all patients, particularly those in underserved populations, deserve access to and assistance with help in navigating the immense and complicated healthcare system. Desiree has worked in healthcare as a RN since 2006. The bulk of her career has been spent in case management and in the intensive care arena. She has acted as a change agent and enjoys helping to pave the way for more collaborative care among physicians and healthcare organizations to improve care for all of those in need. A passionate and dedicated patient advocate, Desiree has found a home at TANDEM365, an organization which is changing the way that healthcare is delivered in the community. Desiree holds a BS degree in Nursing from Ohio University and is currently pursuing her MS in Nursing from Western Governor’s University with an emphasis on leadership and management. In her spare time Desiree enjoys spending time with her husband and two daughters, reading as many works of fiction as she can squeeze in and renovating a pandora’s box of a 100-year old farmhouse.

Lauren Guttersen, LMSW is the Clinical Manager of Social Work services at Tandem365, a community paramedicine and complex medical case management program that currently serves over 1000 older adults in Kent, Ottawa, Allegan, and Kalamazoo counties. She obtained her Masters of Social Work (MSW) at Western Michigan University in 2011. She is currently a Licensed Clinical MSW in the state of Michigan. Lauren has over 10 years of experience working in Community Case Management, Psychotherapy and Residential Behavioral Health roles, most often focusing on the aging population. Prior to her current role with Tandem365, Lauren has gained advanced training in Trauma Informed Care, Substance Use Disorders, Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT). Advocating for and empowering the underserved is her true passion. Lauren has received additional training in Polarity Thinking through the Interprofessional Institute for Polarity ThinkingTM in Healthcare which has given her the skills to see dilemmas that are prevalent in healthcare with a new “lens”, allowing for a more balanced approach to service delivery

Jay Moore, IT Manager has 20+ years of experience in Information Technology. Jay graduated from Davenport University with a degree in network engineering. Jay is primarily focused on leveraging technology to provide tools that improve collaboration, efficiencies, and quality patient care. Jay has spent 8+ years in the field of Healthcare with a special interest in the unique challenges in the home-based setting. Jay has experience in project management and is responsible for all IT systems including the Electronic Medical Record for Tandem365, myUnity.

Jeremy Bass is an Integrated Care Paramedic Case Manager responsible for routine consultation, coordinating with those involved in your medical care, and making home visits as we make progress on YOUR goals. Jeremy won’t be doing this alone; our team of Nurses, Social Workers, and Medical Assistances will be involved regularly for support and guidance. Jeremy believes in transparency and talking about the “why” we do things versus the “just because” mentality. Prior to TANDEM365, he worked on the road as a Paramedic for 10 years. His time on the ambulance was fun and fair to say interesting. Most of his career was at a local ambulance company. In 2017, he came to Life EMS and shortly after moved into the Integrated Care Paramedic position. Jeremy has been working with TANDEM365 for almost 3 years now. He started as an Urgent PRN Medic before transitioning to the Case Management role. He grew up in Comstock Park and now lives in the Cannonsburg area. Jeremy has been married for 7 years, his wife is a schoolteacher, and he has 2 little girls. When Jeremy is not spending time with his girls he enjoys fishing, pretty much whenever and wherever he can.
