Through customized care that is coordinated by our team of healthcare professionals, participants in TANDEM365 have access to the best services and support available with a single point of contact. All of our services are based on an initial assessment and evaluation by our health professionals.
Registered Nurse Navigators
Chronic disease management, collaboration with primary care physicians (PCPs) and specialists, medication reconciliation, continuous monitoring of medication regimen, navigation of the healthcare system, robust support during care transitions, advocacy, and high-level patient education are services we provide. We can accompany patients to PCP appointments so that patients better understand instructions and communicate all pertinent info to the physician. We provide 24/7 access to a Registered Nurse and Integrated Care Paramedics for rapid response to urgent needs.
Social Work Navigators
Advance care planning development, long-term planning, housing, resource connection, safety and security, cognitive testing, caregiver burden screening and assistance, support and assistance with respite needs, patient and caregiver education, and long-term behavioral health monitoring/support services are provided.
Integrated Care Paramedic Visits
We offer rapid response to any non-life threatening, but urgent, medical issues that may arise. We perform lab specimen collection, nebulizers, oxygen therapy, IV fluids, IV medication administration, EKGs, lift assists post falls at home, and any generalized medical concerns that do not warrant the use of the emergency department.
Personal Emergency Response Systems Services
TANDEM365 encourages all participants to accept the personal emergency response systems (PERS) device. This allows our team to respond to falls as well as provide direct access to our triage line for assistance.
Care coordination is one of the keys to TANDEM365’s success. Our expertise ensures that all healthcare providers and individuals involved in our participants’ care stay informed and connected, which reduces the gaps in care and prevents fragmentation.
24/7 Rapid Response Team
As one of the most important components of our program, all participant-related calls are managed by experts, 24/7. After triaging the call, an Integrated Community Paramedic will provide a rapid response to any non-life threatening, but urgent, medical issues that may arise in this patient population. The Integrated Care Paramedic provides interventions determined by a complete physical assessment. Physician orders are signed in advance so that treatment is not delayed, and an emergency room visit can be avoided.

Monitored Medication Dispensers & Medication Management
Many hospitalizations are a direct result of mismanagement of medications. TANDEM365 ensures an accurate and appropriate medication list as well as ensures that the participants have a reliable mechanism of delivery and setup through a monitored medication dispenser, medication delivery system, or med boxes. A medical assistant may be utilized to coordinate these services.
Home delivered meals are intended for participants who are not able to prepare/cook their own meals and without this home delivered service would not receive regular nourishment. This resource is intended for participants who do not qualify for this service through other community programs.
TANDEM365 is committed to ensuring that a participant has regular scheduled appointments with their physician. Transportation may be provided as necessary to ensure that the participant remains compliant.
Chore services are sometimes necessary for participants who need assistance with errands or housekeeping. This resource is intended for participants who do not qualify for this service through other community programs.
Respite care provides temporary relief for a primary caregiver, enabling a much-needed break from the demands of care-giving a sick, aging, or disabled family member.
Personal Care Aid
This service is intended for participants who need assistance with bathing and is offered to participants who do not qualify for this service through other community programs.
Telehealth allows TANDEM365 to remotely monitor a participant’s blood pressure, pulse, and weight. It also allows the participant to manually enter any other measures the nurse may be interested in, such as, blood sugar and pulse oximetry. The participant may also receive questions to answer pertaining to their disease. This data assists the nurse with a snapshot of what is going on with the participant, allowing early intervention to prevent hospitalization.
Participants assess their current physical and emotional state through this resource by setting goals for what they want to achieve in their overall wellness and health and working on plans of action to reach these goals.

TANDEM365’s team of professionals provides limited mental health support; however, we are committed to the care and coordination of our participants who need mental health support. Therefore, we strive to connect to expert providers within the community to create a supportive plan of care.
