Sherry Gosset
Field SupervisorSHERRY GOSSET, RN is a Registered Nurse with Tandem 365. In this role, Sherry coordinates all aspects of Home Case Management including physical and emotional assessment, coordinating care with medical providers, facilitating necessary resources, for needs identified to allow participants to remain in their home and avoid unnecessary emergency and hospital visits. Sherry holds a diploma in Nursing from Blodgett School of Nursing. Sherry has spent 38 years in varied specialties, such as Mother Baby, Pain management, Medical Surgical, Oncology, Orthopedics, Neurology, Progressive Care and Home-based Case Management. Sherry has been with Tandem 365 for 5 years, assisting participants to navigate the health care system and advocating, on their behalf, for their needs. Sherry also holds the role of Field Supervisor which encompasses visits with participants and the supervision of field staff.
Amy Muller
RN NavigatorAMY MULLER, RN received her Bachelor of Science degree in nursing from GVSU in 2001 and has worked full time as an RN ever since. Starting at Blodgett Hospital, Amy worked as an orthopedic/med-surg nurse for 5 years, followed by 2 years in the primary care setting. For the next 7 years, Amy devoted her nursing career to mental health, working as a case manager in residential behavioral health programs, staff nurse in addictions, and as a nursing supervisor in the inpatient psychiatric hospital. Amy has been with Tandem365 since 2015. She values the longevity of the relationships she builds with her participants and considers it an honor to be able to provide support, education, and advocacy for her patients. In her spare time, in addition to raising three school age daughters, Amy loves musicals, movies, and spending time with her many pets.

Field SupervisorCARRIE TOLAND, RN is a Nurse Navigator with Tandem365. In this role, Carrie assists in the health care management of participants in the community with the goal of prevention of hospitalization and falls. Carrie Toland has 10 years of expertise in nursing. Before joining Tandem365 in 2019, Carrie worked as an Emergency Department nurse for 3 years where she has extensive knowledge in multiple disease processes. Prior to the Emergency Department, Carrie worked as a triage nurse for a Primary Care practice and as a Hospice nurse where she has experience with end of life care. Carrie is compassionate and is dedicated to her participants.
Jan Hamstra
RN NavigatorJAN HAMSTRA, RN is a Nurse Navigator with TANDEM365. In her role, Jan assists with the health management of participants in the community with the goal of keeping them in their own homes and avoiding frequent trips to the hospital. Jan has 24 years of expertise in nursing; the majority of which were in roles that focused on the older adult population. Jan is passionate about providing care to patients in their own homes and improving quality of life. Before joining TANDEM365, Jan held other case management roles and has extensive experience in the long term care setting. Jan is compassionate and dedicated to her participants and is an asset to the clinical team.
Jamie Armstrong
RN NavigatorJAMIE ARMSTRONG, RN is an RN on the TANDEM365 team. In this role, Jamie assists in the health care management of participants in the community with the goal of prevention of hospitalizations as well as assisting participants in their personal health goals. She has worked as a paramedic for a local ambulance company before obtaining her nursing degree. Prior to coming to TANDEM365, Jamie worked as a home care nurse for a local agency. In her role as an RN Navigator, she enjoys providing her participants with the tools to manage their health care as independently as possible. Jamie is married with two children. She enjoys spending time with her family, cooking, and traveling.
Kelli Singleton
RN NavigatorKELLI SINGELTON, RN has been a nurse for four years. Kelli has been a supervisor at a rehab and a long-term care facility. Kelli also has worked as a RN case manager in homecare for about three years. Kelli has plans to further her education in the nursing field. Kelli dedicates her free time to the youth in her community by volunteering at two local non profit organizations. She also enjoys traveling, boating, attending church, and going to local sporting events with her husband and children.

RN NavigatorCALVIN STROMBECK, is a Nurse Navigator on the TANDEM365 team. In this role, Calvin enjoys providing his participants with the tools to manage their health care and meet their personal health goals. Prior to coming to TANDEM365, Calvin worked as a chef and a Float Nurse at Bronson Battle Creek Hospital. He has experience in the ED, Cardiovascular and Pulmonary, Critical Care, Mother/Baby, Gastroenterology and Surgical areas of the hospital. Calvin received his education from Kalamazoo Valley Community College and is currently pursuing his Bachelor’s in Nursing. He is married with two young boys and enjoys cooking and being outdoors with his family.

In this role she assists in the health care management of participants in the community with the goal of hospitalization prevention as well as helping them achieve their personal health goals. She has been a registered nurse since 2006. Prior to joining our team, Mickele worked as a charge nurse in an ambulatory surgical setting. She also has experience working as an RN Care Manager with the Mid-Michigan District Health Department. In her free time, Mickele enjoys traveling and spending time with her husband and two children.

Mindy has spent the last nine years of her career focused on the care and health management of medically complex patients. She aims to educate her patients on their disease process for better understanding and management. Mindy looks to help patients live their lives to the fullest. ICP CMMARILEE BRADFORD, EMT-P, is an Integrated Care Paramedic Case Manager with Tandem365. In this role, Marilee works with physicians, nurses, social workers, medical assistants and many other professionals to coordinate the care of Tandem365’s participants. Marilee has more than 22 years of experience as a paramedic. Before joining the Tandem365 team she worked at a local ambulance company providing advanced life support and critical care to the people of her community. She also held leadership roles in the EMS system and once worked as a full-time firefighter. Drawing on her 22 years of experience as a paramedic, Marilee now focuses mainly on case management and the coordination of care in order to keep Tandem365 participants safe in their homes for as long as possible.

ICP CMALEX BURGNON, ICP, is an Integrated Care Paramedic Case Manager with Tandem 365. Alex has 10 years of experience as a paramedic, including roles as a critical care paramedic and field training officer. Alex’s job is to help manage all aspects of his participant’s care and to coordinate with physicians, nurses, social workers and medical assistants in order to formulate the best care plan. Alex will also provide in home assessments and treatments as needed. In their free time, Alex and his wife make it a point to travel as much as possible with an emphasis on the outdoors and exploring other cultures

EMT-PHIEDI JOHNSON, EMT-P, is an Integrated Care Paramedic with Tandem365. She has worked in the Kent County EMS system for over 20 years, including time spent as a Critical Care Paramedic and a Field Training Officer. In her role with Tandem365, Hiedi works with primary care physicians, any specialists, and the participant to help provide the best, most appropriate care, in a timely fashion. She can also provide in home assessments and treatments to help improve outcomes and keep participants safe in their home for as long as possible.

EMT-PHEATHER HOWARD, EMT-P, is an Integrated Care Paramedic with Tandem 365. She has worked in the EMS field for over 20 years. She began her career as a Medic in the Army in 1992 and has been working in Michigan as a Paramedic for 14 years. She is passionate about supporting clients and honoring their choices regarding all aspects of their health. She will work to ensure continuity of care between clients, their families, primary care providers and involved specialists. As a Paramedic she is able to provide this quality case management while being available to address urgent needs of clients. She will perform in-home assessments and, when appropriate, provide treatments which could allow clients to remain in the comfort of their home. Heather’s husband is also a Paramedic and a Veteran. They love to spend time with their 3 children and 1 grandchild.

Kevin has over 12 years providing emergent pre-hospital care including advanced life support care as well as serving in a leadership role as an EMS Director. He now uses his experience to provide home assessments, treatments, and coordinate care in order to keep each participant safe in their home. In his free time, Kevin and his family enjoy many outdoor activities including, fishing, camping, as well as zoos and aquariums.

Joe holds a bachelors in Homeland Security and Emergency Management through the University of Alaska Fairbanks and is currently studying for a Master of Social Work & a Master of Science in Disaster Resilience Leadership through Tulane University. CEOTERESA TOLAND, CEO, is responsible for a startup business where no blueprint or conditions of participation exist. Her 25+ years of experience in community health care has prepared her for her role in this new initiative. Teresa has spent the past eight years getting to know the health care systems and payers in our community and connecting in a positive way. The success of Tandem365 relies strongly on the ability to understand what health care systems, physicians, payers, patients and caregivers need today and being prepared for a dynamic healthcare industry. Teresa firmly believes that healthcare will continue to fail in the effort to delivery high quality care while at the same time manage the cost of delivering that care. Healthcare providers are confronted every day with situations that challenge them to balance patient needs, insurance demands, and regulatory restrictions and yet produce “quality” driven outcomes. Many American’s will be left on the sideline struggling to make sound choices while attempting to understand a very fragmented complex health system.

Medical DirectorMICHAEL MCCULLY, DO, serves as TANDEM365’s Medical Director with over 30 years of experience in both medical and academic sectors. Dr. McCully earned his undergraduate degree at St. Edward’s University in Austin, Texas and his medical degree from Kirksville College of Osteopathic Medicine in Kirksville, MO. He is a long time member of the American Osteopathic Association and the American College of Osteopathic Internists among several others. He also has been awarded many honors including Physician of the Year from Dallas-Ft Worth Medical Center and Internal Medicine Faculty Educator of the Year. Dr. McCully’s comprehensive experience with diverse populations compliments his ability to guide and lead TANDEM365’s clinical initiatives
Tara Wilcox
Director of Clinical ServicesTARA WILCOX, RN, BSN, WCC, is the Director of Clinical Services at Tandem365, a community paramedicine and complex medical case management program that currently serves over 1000 older adults in Kent, Ottawa, Allegan, and Kalamazoo counties. She obtained her BS in Nursing from Grand Valley State University in 2004. Tara has spent most of her career serving the aging population in various roles, but post-acute care in the home setting is her true passion. Prior to her current role with Tandem365, Tara worked to obtain several advanced certifications in wound management, treatments, and debridement so that patients could have access to advanced wound care options in the comfort of their own home. Tara’s current role fits her true passion for the elderly population due to her main interests in bettering the quality and access to care options available to seniors, assisting the at-risk elderly to age in place, and robust advocacy for improvements in navigating older adults through the complex healthcare system.

Chief Operating OfficerPHILIP M FENNEMA, EMT-P, CCP, is the Director of Operations for TANDEM365. TANDEM365 is an innovative organization designed to provide better care, better health, at lower cost. TANDEM365’s primary purpose is to allow older adults to age in place and safely remain in their homes while avoiding unnecessary hospitalizations. TANDEM365 operates in the space where a medical case management program intersects with a community paramedic program. Philip’s 17 years’ experience in emergency medicine provides a unique insight into the challenges an organization like TANDEM365 faces. Philip recognizes the challenges older adults with complex medical conditions face and firmly believes TANDEM365 can provide innovative solutions. Through engagement with local, regional, and state level emergency medical service agencies, Philip serves this integrated care model by ensuring non-traditional utilization of EMS services is effective.
Laura Kasperski
Clinical Manager RNLAURA KASPERSKI, RN, is currently the Clinical Manager of the nursing team at Tandem365, a complex case management and community paramedicine program that serves over 1000 complex older adults in Kent, Ottawa, Allegan, and Kalamazoo counties. Laura obtained her BS in nursing from Mount Mercy College in Cedar Rapids, Iowa and quickly knew that she had a passion for the elderly population. Laura gained pivotal experience in both the ER and ICU settings prior to taking on more leadership roles that specifically influenced the care and quality of life of the aging population. Laura has received additional training in Polarity Thinking through the Interprofessional Institute for Polarity ThinkingTM in Healthcare which she has used to lead and guide her team though difficult situations and more easily adjust to change. Laura continues to strive to be a true change agent and believes that challenging the status quo in healthcare will lead to improvements in collaboration amongst providers, enhance access and quality of life for patients, and impact the overall healthcare system in a positive way.

Operations ManagerJEREMY BASS, ICP CPM, is an Integrated Care Paramedic Case Manager responsible for routine consultation, coordinating with those involved in your medical care, and making home visits as we make progress on YOUR goals. Jeremy won’t be doing this alone; our team of Nurses, Social Workers, and Medical Assistances will be involved regularly for support and guidance. Jeremy believes in transparency and talking about the “why” we do things versus the “just because” mentality. Prior to TANDEM365, he worked on the road as a Paramedic for 10 years. His time on the ambulance was fun and fair to say interesting. Most of his career was at a local ambulance company. In 2017, he came to Life EMS and shortly after moved into the Integrated Care Paramedic position. Jeremy has been working with TANDEM365 for almost 3 years now. He started as an Urgent PRN Medic before transitioning to the Case Management role. He grew up in Comstock Park and now lives in the Cannonsburg area. Jeremy has been married for 7 years, his wife is a schoolteacher, and he has 2 little girls. When Jeremy is not spending time with his girls he enjoys fishing, pretty much whenever and wherever he can.

He is primarily focused on leveraging technology to provide tools that improve collaboration, efficiencies, and quality patient care. Tony spent 7+ years in the field of Long-Term Care and Hospice. He also has experience in Supporting Electronic Medical Record programs which include Vision, Point Click Care and Brightree as well as a strong background using office 365 and SharePoint as an Administrator. Tony has created high end executive reports and dashboards using PowerBI and automated workflows to help automate employee onboarding, terminations and other HR requests.

Quality and Education SupervisorSTEFANIE MCCALLUM, RN, has worked in health care for over 25 years. She worked as a certified nursing assistant in long-term care, home health care, and private duty before returning to school and earning her Bachelor of Science in Nursing in 2008 at Grand Valley State University. Before coming to TANDEM365 in 2016, Stefanie worked in a medical/surgical critical care setting. She has a real passion for education and teaching, whether it’s educating a participant and their family, her fellow colleagues, or passing her knowledge on to future generations of nurses. In her Quality and Education role, Stefanie ensures that TANDEM365 participants receive care that is Timely, Efficient, Effective, Equitable, and Satisfactory.
